PCOD Drilling

Polycystic Ovarian Syndrome (PCOS) is a hormonal disorder among women of childbearing age that results in a lack of balance in estrogen and progesterone hormones. The imbalance in the hormones can cause ovarian cysts—benign ovarian tumors. PCOS has multiple health issues such as irregular menstrual periods, infertility, cardiovascular problems, and physical changes.

Causes of PCOS

The precise reason for PCOS is still not understood, although it is thought that there is a strong genetic and hormonal influence. Family history plays a part in women who have a parent or sibling with PCOS; if they have a mother or sister with the condition, they are more likely to be affected. Also, overproduction of androgens—male hormones which also occur in females—is thought to disrupt ovulation. High insulin levels, which control blood sugar, can also lead to excessive androgen production, which further worsens the symptoms of PCOS.

Symptoms of PCOS usually start shortly after puberty, with differing levels of severity in different individuals. The most frequent sign is irregular menstrual cycles, but other symptoms can include:

Hormonal Imbalances Resulting in Physical Changes:

  • Excessive hair growth (hirsutism) on the face, chest, abdomen, or limbs
  • Reduced breast size
  • Deepening of the voice
  • Thinning or loss of scalp hair

Additional Symptoms:

  • Acne and oily skin
  • Unexplained weight gain
  • Pelvic pain
  • Mood disorders such as anxiety or depression
  • Infertility

Most women with PCOS have metabolic complications like diabetes, hypertension, and hypercholesterolemia, which tend to be related to obesity accompanying the disorder.Diagnosis of PCOS

PCOS diagnosis entails a combination of medical history evaluation, symptom assessment, and laboratory tests. As there is no one test for PCOS, physicians are obliged to utilize the following diagnostic methods:

  • Physical and Pelvic Exam - To examine for swollen ovaries or other evidence of PCOS.
  • Blood Tests - To test hormone levels, such as androgen and insulin levels.
  • Thyroid Function Tests - To exclude thyroid disease.
  • Fasting Glucose Tests - To check blood glucose control.
  • Lipid Profile - To assess cholesterol and triglyceride levels.
  • Vaginal Ultrasound (Sonography) - To get real-time images of the ovaries and uterus.
  • Pelvic Laparoscopy - A very minor surgical procedure in which a small camera is passed through the belly to inspect the ovaries and reproductive organs in detail.

Possible Complications of PCOS

If not treated, PCOS can cause serious long-term health consequences, such as:

  • Hypertension (High Blood Pressure)
  • High Cholesterol
  • Anxiety and Depression
  • Sleep Apnea
  • Endometrial Cancer (Caused by Abnormal Uterine Lining Thickening)
  • Heart Disease and Risk of Heart Attack
  • Type 2 Diabetes
  • Breast Cancer

Laparoscopic Ovarian Drilling (Ovarian Diathermy) for PCOS

Laparoscopic Ovarian Drilling is a surgical procedure that is used to stimulate ovulation in women with PCOS who have not been responsive to weight loss and fertility drugs. Electrocautery or a laser is employed in this operation to obliterate small segments of the ovaries, lowering androgen levels and normalizing ovulation.

Procedure Overview:

The surgery is performed using laparoscopy, a minimally invasive technique requiring a small incision near the belly button.

  • A tiny camera (laparoscope) is inserted to visualize the reproductive organs.
  • Carbon dioxide gas is used to gently inflate the abdomen, providing a clear view for the surgeon.
  • Controlled electrocautery or laser is used on the ovaries to enhance function of ovulation.
  • The operation is performed under general anesthesia for patient comfort.

Post-Operative Recovery

  • Most patients are discharged on the same day and are able to resume normal activities within 24 hours.
  • The healing time is variable, generally ranging from a few days to 2-4 weeks, depending on rates of healing.
  • Mild bloating or discomfort in the abdomen can happen, but they will generally resolve very quickly.
  • The operation is performed under general anesthesia for patient comfort.
